Parent and Child Class
•LESSONS WILL BEGIN THE WEEK OF JUNE 8th.
•Review and agree to all swim lesson guidelines. •Fill out all requested information. •When finished, click on the "Submit Form" button. •If you are having difficulty, please contact our Swim Lesson Coordinator.
◦ Lessons are generally taught two times a week. ◦This Request Form must be complete and submitted at least two weeks before the start date. Please note: only one student per form. ◦Students must be one years of age by June 1, 2015 to participate. ◦
Weather Policy: All participants should arrive at their scheduled lesson time REGARDLESS of the weather conditions. Further information will be provided by the Instructor if the weather does not permit class from being conducted during the scheduled time. When completing the form be sure to include a good working number for the instructor to call.
◦If a weather make up class or an instructor canceled class is not held before the last scheduled day of the class, it will be scheduled the week after the session ends. ◦Up to two classes canceled due to weather will have a make time scheduled. Confirm the dates and times with your instructor. ◦GHPMI will not make up any class that a student misses due to illness, vacation, schedule conflict, etc. ◦Only students are allowed on deck during class times. All parents and guests must remain on the sidelines. ◦All students, parents, and guests are required to follow the pool rules posted at the facility. ◦A swim lesson registration is not final until full payment is received by GHPMI and you receive an e-mail confirmation from the GHPMI office.
*Group Swim Lesson Packages (8 lesson = 30 minutes): $70.00 per student *When paying online each credit card charge will include a $2.00 convenience fee. *When the date and time of the lessons are confirmed you will receive an email with instructions on how to pay online. *Returned checks will require a $30.00 service fee.
Group Swim Lesson Cancellations
*There will be a refund (GHPMI will retain a $15.00 non-refundable administrative fee) given for canceling out of a group lesson prior to 12:00pm on the Friday prior to the first day of class. *There will be a refund (GHPMI will retain a $15.00 non-refundable administrative fee and a $15.00 cancellation fee) given for canceling out of a group lesson after 12:00pm on the Friday prior to the first day of class. *If at the completion of the first day of class you are not satisfied, please contact our Swim Lesson Coordinator via e-mail. *Refund requests must be made in writing. Refunds will not be available after the second day of class. *Credit card payments will be issued a refund to the same card. Cash and check payments will be refunded by check, and will take approximately three (3) weeks to process. Refunds must be requested within 60 days of the first day of class. Cash refunds will not be issued.

As the student/guardian, I verify that the information is accurate and promise to adhere to all swim lesson guidelines stated above. There will be NO exceptions to the above policies. In addition, I fully understand that the student has enrolled in an aquatic program that includes strenuous activity. I affirm that the student is in good physical condition and does not have any disabilities or medical conditions that would limit the student’s participation in the aquatic program. During participation in this program I give permission to have the student treated in an emergency. (Note: Agreement is required by swim lesson student/guardian).
